Intrathecal Pump Billing and Coding

Intrathecal pump billing covers the implant (62360 to 62362), ongoing refills (95990 and 95991) and pump analysis and programming (62367 to 62370), plus the drugs placed in the pump. The refill and programming codes overlap, so choosing the right combination at each visit matters. DOCS4DOCS bills each pump visit with the correct procedure code and drug units.

Billing rules

How do you bill intrathecal pump visits?

Which code do you bill at an intrathecal pump visit?

The code depends on what was done at the visit. A refill alone is 95990, or 95991 with physician skill. Analysis is 62367 without reprogramming and 62368 with it. Analysis with reprogramming and refill is 62369, or 62370 with physician skill, and those two already include the refill.

  • Refill only: 95990, or 95991 when physician skill is required
  • Analysis without reprogramming or refill: 62367
  • Analysis with reprogramming: 62368
  • Analysis with reprogramming and refill: 62369, or 62370 when physician skill is required
  • Do not bill 95990 or 95991 with 62369 or 62370, since those codes already include the refill

How are intrathecal pump drugs billed?

Drugs placed in the pump are billed with their HCPCS codes in the correct units for the amount used. Medicare requires the JW modifier for discarded drug from a single-dose container, or JZ when none was discarded. Drug units that do not match the documented amount are a common reason pump claims are denied.

Common denials

Why are intrathecal pump claims denied?

These are the denial reasons we see most often. We check for each one before a claim goes out, instead of finding out from a denial.

Denials we prevent

  • Refill code billed together with a programming code that already includes the refill
  • Drug units that do not match the amount documented
  • Physician-skill codes billed without documentation of the physician's work

FAQ

Intrathecal Pump Management questions

What is the CPT code for an intrathecal pump refill?

A pump refill without analysis is billed with 95990, or 95991 when it requires a physician's skill. If the pump is also analyzed and reprogrammed, use 62369 or 62370 instead.

Can 95990 be billed with 62369?

No. 62369 and 62370 already include the refill, so 95990 and 95991 are not billed with them.
