Trigger Point Injection Billing and Coding

Trigger point injections are billed with CPT 20552 when one or two muscles are injected and 20553 when three or more muscles are injected. The code is billed once per session based on the number of muscles, not the number of injections. The note must name each muscle injected and the exam findings that support the diagnosis.

Billing rules

How do you bill trigger point injections?

What documentation do trigger point injections need?

Payers expect the note to name every muscle injected, describe the trigger points found on exam, such as a palpable taut band and referred pain, record the drug and amount injected, and show how the patient responded to any earlier trigger point injections.

  • Each muscle injected, by name
  • Exam findings of the trigger points, such as a palpable taut band and referred pain
  • The drug and amount injected
  • Response to any previous trigger point injections

How do you code trigger point injections correctly?

Bill 20552 for one or two muscles or 20553 for three or more, once per session and never once per injection. The injected drug is billed separately with its HCPCS code, and a same-day office visit needs modifier 25 and a separately documented evaluation.

  • Bill 20552 or 20553 once per session, never once per injection
  • Bill the injected drug separately with its HCPCS code
  • A same-day office visit needs modifier 25 and a separately documented evaluation

Common denials

Why are trigger point injection claims denied?

These are the denial reasons we see most often. We check for each one before a claim goes out, instead of finding out from a denial.

Denials we prevent

  • Billing one unit per injection instead of one code per session
  • Muscles injected not named in the note
  • Same-day E/M billed without modifier 25
  • Payer frequency limits for repeat injections exceeded
